Participation Hours Tracking Sheet
A copy of this form is to be maintained by each member and used to document their volunteer hours throughout the year. Each member must complete a minimum of ten (10) hours each calendar year. This form is to be turned in at the end of the year along with next year’s renewal dues payment. Failure to complete and document the minimum of 10 hours will result in the assessment of an additional $100.00 in dues. Participation hours may be verified by any Board Member, Officer, or Event Coordinator.
